User Form

Selecting User Form from Add Page adds a page to the quiz. You can add only one User Form per quiz. The User Form has two sub-items:

• Content — form title, description, and image
• Form Fields — field labels, visibility, and required options

Use the right-panel Settings tab for content and fields, and the Design tab for fonts, colors, and field styling.

How to Add a User Form

1. Open the Quiz section.
2. Click Add Page → User Form.
3. Drag the User Form to the desired position among your questions.
4. Edit Content and Form Fields.
5. Open Design to match your brand.
6. Click Save.

How to Change the Position of a User Form

1. Hover over User Form in the Builder list.
2. Use the drag handle that appears.
3. Drag the User Form up or down to place it before, between, or after your questions.

You can place the User Form before, between, or after your questions.

How to Delete the User Form

1. Hover over User Form in the Builder list.
2. Click the delete icon.
3. Confirm if prompted.

Center Preview

The center preview shows the User Form as users will see it. A typical preview includes:

• Form title, for example User Form
• First Name and Last Name side by side
• Phone with country code
• Email
• Newsletter checkbox, for example Keep me up to date on news and exclusive offers

User Form Content Settings

Click Content under User Form.

Form Title	Main heading shown on the form.
Form Description	Extra information or instructions.

Form Image Settings

User Form Fields

Click Form Fields under User Form. Each field can include:

Control	Details
Label Text	Maximum 40 characters. Customize the label shown to users.
Visibility	Show or hide the field.
Required	When enabled, users must complete the field before continuing.

Disclaimer Message

Setting	Details
Disclaimer Message	Maximum 200 characters. Optional text below the form, such as a privacy notice.

Note:-

• Labels can be renamed to match your store language.
• Hidden fields are not shown to users.
• Required fields must be completed before the form can be submitted.
• Each field can be configured independently.
• The disclaimer appears below the form only when you add text.

User Form Design Settings

Open the Design tab while the User Form is selected. A notice appears stating that styles Apply to all question types.
